Opening of new inquest into Sally’s death …..

Following the High Court ruling on 1 December 2021 ordering the quashing of Sally’s original inquest held in October 2015 and a fresh inquest, the new inquest was officially opened at Hull and East Riding of Yorkshire Coroner’s Court yesterday.

It was agreed that a transcript of the original 8-day inquest will be admitted onto the record and the focus of the new inquest will be on information previously deliberately withheld from the original inquest. This information relates to a conversation which took place between Consultant Psychiatrist, Dr Kwame Opoku-Fofie and Laura Elliot, one of Sally’s Community Psychiatric Nurses. This conversation immediately followed the “unconscionable” and “quixotic” assessment of Sally by the two Crisis Team nurses, Patrick McKee and Gemma Pearson (now Marsh), who refused Sally hospital admission. The inquest will consider, among other things, whether this discussion provided a further opportunity to save Sally’s life.

The inquest is anticipated to take place later this year.
